USE THIS FORM ONLY IF YOU ARE A MEMBER OF THE SETTLEMENT CLASS TO MAKE A CLAIM FOR A CASH PAYMENT OR UNREIMBURSED LOSSES.


GENERAL INSTRUCTIONS


If you are a member of the Settlement Class, you are eligible to complete this Claim Form to claim (1) a cash payment or (2) up to $5,000 for reimbursement for documented out-of-pocket expenses.


Please refer to the Notice for more information on submitting a Claim Form and information on the aggregate cap on claims.


To receive any of these benefits, you must submit a Claim Form by February 4, 2025.


To start your online claim, you can find your Notice ID & PIN printed on the postcard Notice that was mailed to you. If you are unable to locate your ID & PIN, please contact us by emailing info@SheffieldSettlement.com. Please request your Notice ID & PIN by providing the Class Member’s full name and mailing address.
